Spolu se \u201eSlon\u00edm mu\u017eem\u201c Josephem Merrickem jde o dva z mnoha zn\u00e1m\u00fdch p\u0159\u00edklad\u016f formuj\u00edc\u00edch popul\u00e1rn\u00ed a \u010dasto myln\u00e9 p\u0159edstavy v souvislosti s t\u00edmto onemocn\u011bn\u00edm. Dlouh\u00e1 desetilet\u00ed si toti\u017e lid\u00e9 mysleli, \u017ee ob\u011b tyto postavy trp\u011bly pr\u00e1v\u011b t\u00edmto onemocn\u011bn\u00edm. Teprve v roce 1882 Frederick Daniel von Recklinghausen rozlouskl jeho genetickou podstatu a polo\u017eil z\u00e1klady pro dal\u0161\u00ed v\u00fdzkum a lep\u0161\u00ed porozum\u011bn\u00ed tomuto onemocn\u011bn\u00ed.&nbsp;<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Neurofibromat\u00f3za je d\u011bdi\u010dn\u00e9 onemocn\u011bn\u00ed vyzna\u010duj\u00edc\u00ed se tvorbou n\u00e1dor\u016f formuj\u00edc\u00edch se z nervov\u00e9 tk\u00e1n\u011b. V\u011bt\u0161inou jsou tyto n\u00e1dory benign\u00ed (nezhoubn\u00e9), av\u0161ak n\u011bkdy mohou b\u00fdt malign\u00ed (zhoubn\u00e9) a metast\u00e1zovat. V z\u00e1vislosti na mutovan\u00e9m genu a klinick\u00fdch p\u0159\u00edznac\u00edch rozli\u0161ujeme 2 typy tohoto onemocn\u011bn\u00ed, a to neurofibromat\u00f3zu typu 1 (NF1) a neurofibromat\u00f3zu typu 2 (NF2).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong>Ne v\u017edy za ni mohou na\u0161i p\u0159edkov\u00e9<\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Neurofibromat\u00f3za typu 1, stejn\u011b jako neurofibromat\u00f3za typu 2, se d\u011bd\u00ed autosom\u00e1ln\u011b dominantn\u011b, co\u017e znamen\u00e1, \u017ee k rozvoji onemocn\u011bn\u00ed sta\u010d\u00ed p\u0159\u00edtomnost mutace v&nbsp;jedn\u00e9 ze dvou verz\u00ed dan\u00e9ho genu. V p\u0159\u00edpad\u011b, \u017ee t\u00edmto d\u011bdi\u010dn\u00fdm onemocn\u011bn\u00edm trp\u00ed jeden z&nbsp;rodi\u010d\u016f, riziko, \u017ee onemocn\u011bn\u00ed postihne i jejich d\u00edt\u011b, je 50 %. Pr\u00e1v\u011b neurofibromat\u00f3za typu 1 p\u0159edstavuje nej\u010dast\u011bj\u0161\u00ed typ neurofibromat\u00f3zy zap\u0159\u00ed\u010din\u011bn\u00fd mutac\u00ed v genu NF1 k\u00f3duj\u00edc\u00edm protein neurofibromin. Zaj\u00edmavost\u00ed je, \u017ee gen NF1 lokalizovan\u00fd na 17. chromozomu pat\u0159\u00ed mezi nejdel\u0161\u00ed geny lidsk\u00e9ho genomu, pravd\u011bpodobn\u011b d\u00edky \u010demu\u017e m\u016f\u017ee \u010dast\u011bji doch\u00e1zet ke vzniku nov\u00fdch spont\u00e1nn\u00edch mutac\u00ed je\u0161t\u011b p\u0159ed po\u010det\u00edm v&nbsp;pohlavn\u00edch bu\u0148k\u00e1ch. Pr\u00e1v\u011b z tohoto d\u016fvodu a\u017e polovinu p\u0159\u00edpad\u016f NF1 tvo\u0159\u00ed pacienti bez pozitivn\u00ed rodinn\u00e9 anamn\u00e9zy, a tud\u00ed\u017e mutaci nezd\u011bdili od jednoho z rodi\u010d\u016f, ale vznikla u nich nov\u011b.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Naopak neurofibromat\u00f3za typu 2 je mnohem vz\u00e1cn\u011bj\u0161\u00ed forma neurofibromat\u00f3zy zap\u0159\u00ed\u010din\u011bn\u00e1 mutac\u00ed v genu NF2 lokalizovan\u00e9m na 22. chromozomu, kter\u00fd je zodpov\u011bdn\u00fd za produkci proteinu merlin. Stejn\u011b jako u NF1 existuje 50&nbsp;% pravd\u011bpodobnost p\u0159enosu posti\u017een\u00e9ho genu z rodi\u010de na potomka a t\u00e9m\u011b\u0159 50 % p\u0159\u00edpad\u016f se p\u0159ipisuje nov\u011b vznikl\u00fdm mutac\u00edm.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong>KLINICK\u00c9 P\u0158\u00cdZNAKY&nbsp;<\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Nej\u010dast\u011bj\u0161\u00edm projevem neurofibromat\u00f3zy typu 1 jsou mnoho\u010detn\u00e9 n\u00e1dory nerv\u016f a k\u016f\u017ee, tzv. neurofibromy, jako\u017e i oblasti se sv\u011btle hn\u011bdou barvou k\u016f\u017ee (tzv. caf\u00e9-au-lait). Takov\u00e9 abnormality pigmentace k\u016f\u017ee se \u010dasto vyskytuj\u00ed p\u0159i narozen\u00ed nebo v prvn\u00edch letech \u017eivota d\u00edt\u011bte, p\u0159i\u010dem\u017e maj\u00ed tendenci nar\u016fstat co do velikosti a po\u010dtu b\u011bhem d\u011btstv\u00ed. Krom\u011b ji\u017e zm\u00edn\u011bn\u00fdch p\u0159\u00edznak\u016f m\u016f\u017eeme pozorovat tak\u00e9 skoli\u00f3zu, zv\u00fd\u0161en\u00fd krevn\u00ed tlak \u010di p\u0159\u00edtomnost n\u00e1doru zrakov\u00e9ho nervu.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">P\u0159esto\u017ee se u NF1 vyskytuj\u00ed n\u00e1dory \u010dast\u011bji na povrchu t\u011bla a nervov\u00fdch tk\u00e1n\u00edch, neurofibromat\u00f3za typu 2 se projevuje zejm\u00e9na n\u00e1dory na nervech, kter\u00e9 ovliv\u0148uj\u00ed sluch a rovnov\u00e1hu. U tohoto typu onemocn\u011bn\u00ed m\u016f\u017eeme pozorovat zvon\u011bn\u00ed v u\u0161\u00edch, \u0161patnou rovnov\u00e1hu, bolesti hlavy \u010di epileptick\u00e9 z\u00e1chvaty. Nav\u00edc n\u00e1dory na sluchov\u00fdch nervech mohou zp\u016fsobit postupnou ztr\u00e1tu sluchu. Obecn\u011b se klinick\u00e9 projevy objevuj\u00ed b\u011bhem pozdn\u00edho dosp\u00edv\u00e1n\u00ed a ran\u00e9 dosp\u011blosti, p\u0159i\u010dem\u017e jde o vz\u00e1cn\u011bj\u0161\u00ed formu s hor\u0161\u00ed progn\u00f3zou ne\u017e u NF1.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong>PRO\u010c JE DOBR\u00c9 ZN\u00c1T TUTO PREDISPOZICI?<\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Vzhledem k tomu, \u017ee neurofibromat\u00f3za je v\u00fdlu\u010dn\u011b geneticky podm\u00edn\u011bn\u00e9 onemocn\u011bn\u00ed, faktory prost\u0159ed\u00ed a \u017eivotn\u00ed styl nemaj\u00ed vliv na v\u00fdvoj tohoto onemocn\u011bn\u00ed. Zn\u00e1t svou predispozici pro rozvoj tohoto onemocn\u011bn\u00ed m\u00e1 smysl z n\u011bkolika d\u016fvod\u016f:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Prevence a monitorov\u00e1n\u00ed \u2013 znalost genetick\u00e9 predispozice pro rozvoj neurofibromat\u00f3zy umo\u017e\u0148uje l\u00e9ka\u0159\u016fm identifikovat p\u0159\u00edznaky a za\u010d\u00edt s monitorov\u00e1n\u00edm zdravotn\u00edho stavu mnohem d\u0159\u00edve. P\u0159esto\u017ee tento typ onemocn\u011bn\u00ed nelze zcela vyl\u00e9\u010dit, p\u0159\u00edznaky se vhodnou l\u00e9\u010dbou daj\u00ed zm\u00edrnit a t\u00edm se d\u00e1 zlep\u0161it kvalita \u017eivota pacienta.<br><\/li>\n\n\n\n<li>Riziko pro potomstvo \u2013 znalost vlastn\u00ed genetick\u00e9 predispozice umo\u017e\u0148uje vyhodnotit rizika p\u0159enosu tohoto onemocn\u011bn\u00ed na potomstvo. Tato informace m\u016f\u017ee b\u00fdt nav\u00edc d\u016fle\u017eit\u00e1 i pro ty, kte\u0159\u00ed nemaj\u00ed rodinnou historii onemocn\u011bn\u00ed, proto\u017ee neurofibromat\u00f3za \u010dasto vznik\u00e1 spont\u00e1nn\u011b, bez p\u0159edchoz\u00edho v\u00fdskytu v rodin\u011b.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong>Nov\u00e1 predispozice je ji\u017e ve va\u0161\u00ed anal\u00fdze<\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">V\u00fdsledek spolu s dal\u0161\u00edmi informacemi ohl\u011bdn\u011b neurofibromat\u00f3zy je odte\u010f zdarma dostupn\u00fd pro v\u0161echny na\u0161e z\u00e1kazn\u00edky.
